Required Forms for New Students

The Office of Student Affairs mails out Health, Roommate Matching, and Housing Agreement forms to all new students. In addition, these forms, in PDF format, can also be downloaded via the links below. Students who do not enroll in Mitchell College's insurance program must complete an insurance waiver form.
