Transcript Request
Your Mitchell College transcript can be sent to the person/location of your choice by filling out a transcript request form in the Registrar's Office (Mitchell Hall) on campus or by mailing the below listed items to:

A. Background Information = you must submit the following in writing:
* Your name(s) (as it was during your time of attendance)
* Your name today (if different from when you were at MC)
* The year(s) you attended Mitchell College
* Your Social Security Number
* Your date of birth
* Your current address
* Your current phone number
B. Transcript Destination = you must submit the following in writing:
* Name of the Person/School/Business to which you want your transcript sent
* Address of the Person/School/Business to which you want your transcript sent
C. Payment = you must submit the following:
* $5.00 per transcript requested (make check payable to "Mitchell College"
* NOTE: credit cards CAN be used by contacting the Bursar at 860-701-5061 or via email at brennan_l@mitchell.edu to coordinate payment in advance.
D. Signature = you must submit the following:
* Your signature must be part of your request (or we can't send your transcript!)
